Boise school district (BSD) Illness PROCEDURE
Information for if you are experiencing symptoms of illness
ATHLETIC PHYSICAL FORMS

All incoming 7th grade students are required to have a school physical completed AFTER MAY 1st prior to entering 7th grade. This will count for PE and if your student chooses to participate in a school sport.
If your student chooses to participate in school sports they will have to have another school physical again in 9th grade and their 11th grade year.
All incoming 7th grade students are required by the state of Idaho to have TWO immunizations before entering the 7th grade, which are the Tdap and Meningococcal vaccines
If you choose not to immunize your child, you MUST have an exemption form on file for these vaccines.
HEALTH HISTORY
As parents or guardians having you fill out the Student Health History form gives the Nurse's (Health) Office permission to give:
~Cough Drops ~Ibuprofen ~Tylenol ~Tums
Without this form in our records, we cannot give them medication. This form is filled out EACH year for any students attending a BSD school.
***Note: We do not have cold medicines, cough medications, allergy medications, bottled water, or anti-nausea medications available in the Health Office
