🩺 Central District Health Sports Physicals & Immunization Clinic

Riverglen Junior High School is partnering with Central District Health (CDH) to host an on-site Sports Physical and Immunization Clinic to help get student-athletes cleared and fully vaccinated for the upcoming school year!

📅 Clinic Details

• Date: Tuesday, August 4th

• Time: 9:00 AM – 3:00 PM

• Location: Riverglen Junior High School

• Admission: First Come, First Served (No pre-scheduled appointments required)

📋 What to Bring:

1. Parent/Guardian: Students must be accompanied by a parent or legal guardian.

2. Immunization Records: Please bring a copy of your child's current immunization history.

3. Completed Forms: Bring a printed and completed Boise School District Sports Physical Exam Form. You can download Athlete Physical forms here.

4. Cost: $25 payable by cash, check or credit card.
